I think you may have difficulties finding a replacement as it is so old.
There are various obsolete and salvaged electrical parts companies out there and so it may be possible to source one, ebay is also another method of searching. Mostly the parts are circuit breakers and fuse holders but you may get lucky.
Some plastic adhesive and a bit of plastic polishing may be a solution?
If all else fails then a call to a small plastics fabricator might resolve things, though it may end up easier to change the board!
The cover also looks somewhat similar to the old MEM covers but it is likely they would not be a dimensional match, but might be worth investigating.
 
It was clearly faulty before as you can see it has been stuck on, to the first time I've seen that! Unless it was you? If it wasn't then I would be telling them it was already like that.
 
If you cannot get one easily ? then you may have to repair the old one with some plastic and some glue. You cannot be expected to be responsible for old brittle stuff that breaks easily.
 
I'd agree, its old and brittle. But a simple fix would be a rectangle of clear plastic and just glue it inside the old cover to support it.
