The only forms I've used are NIC ones.

I've attatched examples of the green ones that anyone can use......
 
Hi ,have been asked to carry out periodic inspection of church and seperate church hall.no problem however they have also asked me to test smoke alarms (mains wired interlinked) and the emergency lighting(stand alone units) and provide certificates for fire officer. I have managed to find some certificates on the net for emergency lighting but nothing that seems suitable for the smoke detectors,anybody point me in the right direction? cheers Paul

just print your own with all the results on, as long as you have a record then it proves you have done the required tests, and covers your arse
