Anybody got the danger notice forms as a pdf?

Need to issue a danger notice to a customer. They have a new electric shower without a earth conductor, very low IR readings and to top it off the cable has signs of burning on the damaged earth cable in the back of the isolator. that has been cut off short and is covered in melted green/yellow sleeving
The new high power electric shower was installed by the plumber!
used to be able to fill one in on the NICEIC website but it seems to have disappeared.
Can anybody please send me a pdf version to fill in?

Thanks all, much appreciated. i didnt want to have to fork out for a pad of them since i dont have to issue them all to often thankfully. But every so often you need to cover your backside as i can almost guarantee the customer will carry on using the shower as soon as i am out the front door and i dont want it on my toes if she kills herself!
