I have had differing opinions on this, some niceic assessors tell you to list all the circuits and protective devices, leaving the results blank obviously and a note in the relevant box explaining the work done. Other times i have recorded just the one new circuit leaving the rest out and they have been fine with it.
In your case though from the screen grab it could lead to a confusing certificate for joe public. At the very least i would put upstairs socket, cooker, shower etc in the description boxes and just be lcear on the EIC what it is you have worked on. even if you record nothing else in the subsequent boxes. it clarifies things.
 
On a EIC for an existing installation, I fill in "used" where the MCB is connected and "spare" where an MCB is unused or its a spare way
 
Just come off the phone to the NICEIC (it was a long call...) and their preference is for the screenshot I attached but with the addition of RCD1 and RCD1 being specified for clarity, i.e. all circuits not worked on should be specified as such and only those worked on should be identified specifically.
 
One line on the SOTR with the circuit reference will do for me. Sod filling in every line on a 72 way for a single new circuit.
I also only insert the circuit(s) I've worked on and my NIC Area Engineer is fine with it (but why wouldn't he be?)

I certainly wouldn't waste time filling in details that are irrelevant to the work I've done.
