HI,

I have an extension cable my Dad made years back that looks like this:

How can I attach this to my iPad charger which looks like this:

You will also blow up your phone if you are lucky enough to get that far. the ipad requires 5v not mains at 230v

The Ipad charger "plug" safely drops the voltage to 5v

It is not do-able
look at the diagram you supplied it even says 5v.
Mains voltage is also AC and much higher 230v plus.
The end of the charger would be at mains potential then anyone who touched this would get a shock probably fatal.
 
in the top picture, you need to pull a green and a black wire down inside the sheath, all the way through, as you need 4 wires, and you only have two at the moment.
 
Go into a mobile shop and say I want one of these then chuck the rest in the bin.

Of course the cable won't work. its missing a clamp screw. Duh...
