I put grease on the arbor thread and mating surface and just separate them while it's still in the drill with water pump pliers.
 
Depends what I’m drilling through, but normally I just hold the saw in my left hand and reverse my drill. I then poke out disc with the drill bit, offer up the saw and forward my drill to do up the saw.
