Complaint procedure, sample plz

Just Joined Stroma, they need a complaint procedure but I can seem to find a sample,Can anyone help me out

They need to see you have a "process" or "form" to record complaints, and as such just copy the NICEIC form ............ just don't have any logo's so when you change schemes later, the same form will still be OK to use ............
