I’m in escrow on a 1977 octagon home and my inspector said that he has never seen a panel like this before (he did not look inside of it since he is not an electrician And it’s bolted shut pretty good). What is something like this going to cost to replace? He “guessed” 4k.

It might not need replacing, get a qualified electrician to report on its condition and if it is fit for future service or in need of replacement.
 
Doesn't look as bad the right way up,
As above, it may not need replacing, get a full report from an Electrician.
My Son bought a house recently in Arizona and the seller paid for some of the defects, found on the Home Buyers report, to be corrected.
I believe it was around $5000 worth

I’ve been doing electrical close to 40 years and I have never seen a panel like. I seen a 400 amp breaker so I’m guessing it is a 400 amp service but the breakers are exposed to the elements. Let’s hope that’s a NEMA 3 enclosure. Something like that you just rip it all out and start from scratch. It probably would be around $ 5000.00
 
I've seen similar. It has trough on the right side. On the left top half, there is windows for additional meter socket. Bottom left has the main electrical panels. You would ned to buy additional kits for the covers to accomodate the other meter socket, etc. These are out dated & maybe against the current codes for wire fill & such. But it may not HAVE to be changed.
As stated above, I would defer to a qualified electrician to have a look. I do know Cali is a license state, as it appears from your info, that's where you reside. As far as price, It could be $4,000. Generally speaking, it's $10/amp for a new service.

I reckon also that that set-up shouldn't be exposed to the elements if that it the case or is this actually indoors or undercover and we just have some leaves blown in, the rust would suggest otherwise and that main switch is deffo not IP rated even if the enclosure was.

It's a 400A Square D switchboard, but being a NEMA type 1 enclosure exposed to the outdoors is not good. The upper section has current transformers for metering, the section on the right with the handles is a underground pull box, is it single or 3 phase? There is a label but was covered over by a housewife paint job.?
